(a) Dental Crowns:-
A crown, or full-coverage restoration (sometimes incorrectly called a cap) is a prosthetic tooth designed by a dentist and usually created by a lab technician (or more recently, a CAD-CAM machine). Crowns can be either metal, porcelain fused to metal (PFM), or all porcelain/ceramic. Metals can either be noble metals or base metals, noble metals having higher content of gold, platinum or palladium. Noble metals are typically more accurate when they are cast by the lab technician and offer a better bond with porcelain. Noble alloys in PFMs have a much lower incidence of allergic reactions as they do not contain nickel. All porcelain restorations can be made from feldspathic (traditional) porcelain, lithium disilicates, aluminous porcelains, or zirconia. All ceramic restorations typically are not as strong as those with metal substructures and can cause increased wear of the opposing teeth.
The teeth to be crowned are prepared which involves reduction of the tooth size (usually under local anaesthesia) followed by an impression or mould of the prepared tooth. This trimming of the tooth is required to create space for the crown to be fitted. The mould taken is then sent to a laboratory where skilled technicians will fabricate the crown. In the meanwhile, a temporary crown is made and fitted onto the trimmed tooth.
(b) Dental Bridges:-
A bridge stabilizes the bite of a patient who is missing one or more teeth. Bridges prevent the surrounding teeth from moving or shifting in the mouth. A missing tooth that is not replaced may cause surrounding teeth to become unstable and require removal, compromising oral health or change the shape of the face and diminish the beauty of a smile.
Unlike partial dentures, bridges are permanent; patients need not remove them. The missing tooth is replaced with an artificial tooth connected between two crowns (caps), which are permanently cemented or bonded on the adjacent teeth.
Bridges can be used to replace a small number of missing teeth if the neighboring teeth are sufficiently strong. The number of missing teeth, condition of the neighboring teeth, condition of the supporting gums and bone are all important factors which need to be assessed prior to making a bridge.
(b.1) Dentures:-
(also known as dental plates), can be defined as a set of artificial teeth, which are used when a patient has lost real teeth on the mandibular arch, the maxillary arch, or both. Patients can become entirely edentulous (without teeth) due to severe malnutrition, genetic defects such as Dentin genesis imperfecta, ineffective oral hygiene or trauma. Dentures can help give the edentulous patient better masticatory (chewing) abilities, as well as enhance their aesthetic appeal by providing the illusion of having natural teeth, providing support for their lips and cheeks, and correcting the collapsed appearance commonly seen between the nose and the chin.
(b.3) Removable partial dentures:-
are for patients who are missing only some of their teeth on a particular arch. Fixed partial dentures, better known as crowns and bridges, are also for patients missing only some of their teeth, but these are more expensive than removable appliances, and they are contraindicated in certain instances.
Conversely, complete dentures are worn in patients who are missing all their teeth in an arch (i.e the maxillary or mandibular arch).
